Third Party Authorization & Waiver Form
Our standard age limit for guests is 21, however, Military, Hospital and University Guests under 21 can book a room with authorization from their institution. Parents can also book a room on behalf of their children 18 to 21 years old by completing a Third-Party Authorization & Waiver Form.
In order to expedite the check-in process, we request that the completed form and copy of your driver’s license and credit card must be received by the Hotel, prior to the Guest check-in.
No outside guests allowed for under 21 guests.
No more than 2 adults can stay in one room as well.
Third-Party Authorization & Waiver Form can be filled out and submitted in two ways:
1. Download a PDF form here, print it, or fill in the blanks on your Computer. After filling the form, send us the scanned copy of the form along with your driver’s license and credit card (Both Sides) at GuestServices@SuperiorStayHotel.com.

2. If you are familiar with Google Forms and have a Google ID, you can simply complete an Online Form here Third Party Authorization & Waiver Form, and attach the photo of the required documents (your credit card and driver’s license).
